| We are a consult service providing two distinct services for referring doctors and their patients with chronic pain.
The first is a procedural consult service where the referring doctor requests a diagnostic or therapeutic nerve block or other procedure. For these patients follow-up is provided by the referring doctor. The second service is a comprehensive pain management service that provides procedural, medication, psychological, and rehabilitative treatment for the management of various chronic pain conditions, including neck pain, back pain, complex regional pain syndromes, and cancer. For both services close communication with the referring physician is maintained so that patients may receive the best possible organized care. |
Make an AppointmentIt is imperative that we coordinate our treatments with those of your referring doctor. Thus, to make an appointment you must first have a referral from one of your doctors, preferably from the doctor currently treating your pain problem.Once we have a request from your doctor a patient questionnaire will be provided to you. Please fill out the questionnaire and return it along with all pertinent medical information including MRI, X-Rays, blood test, and list of all medications to our office. |
