Interventional ProceduresMake an AppointmentIt is imperative that we coordinate our treatments with those of your referring doctor. Thus, to make an appointment you must first have a referral from one of your doctors, preferably from the doctor currently treating your pain problem. Please fill out the questionnaire (below) and return it along with all pertinent medical information including MRI, X-Rays, blood test, and list of all medications to our office.
